چکیده

Background: Buprenorphine, a partial agonist of the mu-opioid receptor, is an increasingly prescribed medication for maintenance treatment opioid use disorder. When this taken in context active use, precipitated withdrawal can occur, leading to acute onset symptoms. Fentanyl complicates buprenorphine, as it slowly releases from body stores and lead higher risk withdrawal. Objectives: Describe successful management buprenorphine fentanyl with high doses buprenorphine. We seek highlight how no adverse effects occurred patient illustrate his stable transition outpatient treatment. Case report: present case severe disorder who presented moderately after taking non-prescribed buprenorphine-naloxone which daily use. He was treated 148 mg over first 48 hours, averaging 63 per day four days. The reported rapid improvement symptoms without noted side able successfully taper 16 twice by discharge. Conclusions: This demonstrates safety effectiveness at withdrawal. While other options include symptomatic management, initiating methadone or less researched like ketamine, utilizing preserve re-establish confidence life-saving medication. also increases previously documented upper boundary on dosing should provide additional its use.

Buprenorphine and opioid antagonism, tolerance, and naltrexone-precipitated withdrawal.

The dual antagonist effects of the mixed-action μ-opioid partial agonist/κ-opioid antagonist buprenorphine have not been previously compared in behavioral studies, and it is unknown whether they are comparably modified by chronic exposure. To address this question, the dose-related effects of levorphanol, trans-(-)-3,4-dichloro-N-methyl-N-[2-(1-pyrrolidinyl)cyclohexyl] benzeneacetamide (U50,488...

Severe opiate withdrawal in a heroin user precipitated by a massive buprenorphine dose.

By diverting his dispensed medication, our patient collected 11 buprenorphine tablets (8 mg each), which he took in one day. The result was not respiratory depression, but instead severe opiate withdrawal lasting four days--this scenario has not previously been reported. This case highlights features of the unique pharmacology of buprenorphine and some key issues for its use in the management o...

The onset of withdrawal occurred 48 hours after the last dose, peaked around the third day and lasted up to ten days. The withdrawals ...
